Plasma Exchange
Inuspheresis® and Advanced Therapeutic Plasma Exchange
Inuspheresis® is a specialized form of therapeutic apheresis designed to filter pathologically relevant substances directly from blood plasma using targeted double-filtration technology. The procedure separates plasma from cellular blood components, passes it through specialized intelligent membranes to scrub targeted inflammatory proteins, heavy metals, environmental toxins, and autoantibodies, and then returns the purified plasma back to the patient.

Inuspheresis® vs. Therapeutic Plasma Exchange
Both Inuspheresis® and Therapeutic Plasma Exchange (TPE) fall under the broader umbrella of extracorporeal blood purification therapies. While both aim to remove pathogenic substances from circulation to reduce systemic inflammation, they differ fundamentally in how they handle liquid plasma.

| Feature | Inuspheresis® | Therapeutic Plasma Exchange (TPE) |
|---|---|---|
| Primary Mechanism | Selective double-filtration: Filters and returns the original plasma, which only removes substances with specific molecular weights. | Complete plasma replacement: Removes all harmful substances if they exist in the plasma, regardless of molecular weight, and replaces with albumin. |
| Type of Replacement Fluid | None: Patient retains their own liquid plasma. | High: Requires replacement with human 5% albumin. |
| US Availability | Unavailable in mainstream US clinical practice. | Widely accessible across medical centers and specialized US clinics. |
| Clinical Footprint | Primarily offered in specialized European longevity & functional clinics. | Decades of US ASFA-guided protocols and off-label use for other modalities. |
Advantages of Therapeutic Plasma Exchange
Comprehensive Pathogen Removal
Since TPE completely removes the patient’s existing plasma and replaces it with albumin, after which within a few days the body replaces the albumin with the patient’s own new plasma, it guarantees the complete elimination of circulating inflammatory mediators, autoantibodies, cytokines, and microclots which exist within the patient’s plasma volume. Double-filtration methods are constrained by filter pore sizes, whereas TPE provides a complete replacement of the intravascular fluid environment.
Decades of Clinical Validation
TPE is backed by extensive clinical literature and formal guidelines established by the American Society for Apheresis (ASFA). Indications, volume calculations, and safety parameters for TPE are standardized globally, whereas clinical evidence for Inuspheresis® and similar procedures generally relies heavily on proprietary observational data and smaller private clinic cohorts.
US Accessibility & Regulatory Approval
TPE is a well-established, FDA-approved for certain disease states, readily available in hospitals, academic medical centers, and specialized outpatient clinics across the United States. In contrast, Inuspheresis® is primarily restricted to proprietary clinics in Europe (most notably Germany and Switzerland), making TPE far more accessible without the immense burden and expense of international travel.
Lower Risk of Filter Membrane Clogging
Double-filtration procedures like Inuspheresis® rely on cascading membrane filters that often saturate during high-viscosity or high-protein loads, requiring session pauses or membrane swaps. TPE is not affected by this limitation because it is a complete removal of the plasma.
